Make a Referral
For more information or to make a referral to the Developmental Disabilities program, please contact your closest DePaul Community Resources location.

Most of our referrals come from Community Service Boards and State Training Centers. Often, families will call to gather information and compare services. We are always willing to sit with you and discuss our services.

Once you have decided that you would like to work with DePaul we will guide you each step of the way.

Step One: Step one involves a brief overview of the individuals needs.
This can be done on the phone or with an office visit. Questions based on type of services desired and the personal, emotional and physical care need by the individual will be asked.

Step Two: Involvement of the individual's case manager.
An individual without a case manager from the Community Service Board will be referred to them for an Intake. At that time the individual will work with the CSB in determining need and the possibility of placement on a Medicaid Waiver waiting list. Individuals that have Medicaid waiver funding can begin the process with the case manager's involvement.

Step Three: Gathering information.
Often this information can be received from the CSB Case Manager. Social History, Psychological, Level of Functioning and any current information that would help us determine need is recommended. The DePaul Family Services Application Packet should be completed at this time by someone who knows the individual well. In each step we will be accessing service appropriateness. If at any time we feel we are unable to serve the individual, you will be notified.

Step Four: Matching Services
Once the information is received by our Admissions Social Worker, we will begin reviewing current homes and/or day support centers for openings ensuring that the most appropriate match is made. Once an appropriate match is made, you will be contacted regarding homes and/or day support availability.

Step Five: Visitation & Tours
From there you will have opportunities to meet with families and/or tour day support centers. Prior to an individual moving into a Care Provider home or attending day support, trial visits are completed giving the individual the ability to opt out at any time. If the individual is in agreement for services, funding requests will be completed by our Admissions Social Worker and the CSB Case Manager. If more time is needed to get to know the Care Providers, individuals can choose to visit with the Care Provider over an extended period through our Respite Services.

Fees for Services
Medicaid Waiver Funds:
Our fees are determined by Medicaid. All individuals receiving funding through Medicaid Waiver are billed to Medicaid at the current rate.
