COMMUNITY-BASED MENTORING
The goal of this service is to improve a youth’s daily functioning.
Each youth is carefully matched with a Mentor, who serves as a positive role model. Services are designed to meet the needs of individuals who have moderate emotional or behavioral issues, developmental delays, physical disability, or whose living situation is such that they would benefit from such a service.
Each mentor is specially trained to understand the unique needs of children and adolescents who struggle with emotional and behavioral challenges and are dedicated to helping the child find natural support systems within their own communities.
Features of the program include:
- Individualized Service Plan created with input form the child, family, referring agency, and mentor
- Detailed monthly reports for the referring agency
- Small caseloads for each mentor
- Linking with “natural” support systems in the community to enable the child’s progress to continue after mentoring services have ended
